MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs.
This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes.
Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.
Registered Nurse RNRecent graduates are encouraged to apply!No night or weekend requirements!Mentor Adult Day Health is seeking a Registered Nurse to provide services, training, and support to address the medical and behavioral needs of individuals.
We are a day program and our hours are Monday - Friday from 8am - 4pm.
Responsibilities may include but are not limited to:
Conduct a complete assessment of each participantDevelop individualized care plansOversee the management of high-risk medical conditionsMaintain contact with primary care physicians and specialistsParticipate in quality improvement and educational programsTrain support staff in daily functions within their roles
Qualifications:
