Clinical Practice Transformation Specialist

Adventist HealthCare seeks to hire an experienced Clinical Practice Transformation Specialist who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing.
As a Clinical Practice Transformation Specialist, you will:
o Be responsible for developing a productive relationship with the Adventist HealthCare Physician Alliance Clinically Integrated Network (CIN) practices to achieve Adventist Healthcare's strategic goalso Provide education and guidance related to commercial payor programs utilizing advanced analytics, gaps in care files, and physician score cards to identify patients with high risk and rising risk conditions.
Support practices in closing quality gaps, including patient outreach.
o Collaborate with CIN staff, care navigators, and other support teams to develop strategies and processes for managing attributed patient populations, closing quality gaps, and improving overall practice performanceo Engage CIN practices in practice transformation by serving as primary leader for developing and utilizing workflows tailored to the physician's and practice's needs.
Contribute and lead practice transformation improvement projectso Assist in communicating key data trends for the purposes of strategy in managing risk stratification patient populations and quality metric gapso Participate in the Adventist HealthCare Physician Alliance CIN and its subcommittees and other required meetingsoProvide technical assistance to the practices, serving as a key resource in practice's transformationQualifications include:
o Bachelor's degree in Computer Science, Health Informatics, Healthcare Administration or equivalent, MBA or MHA preferredo 2-3 years of experience required, preferably in a healthcare settingo Work experience in a physician practice is preferredo Ability to work in high volume environment.
